I killed a mob, when it died I got a handler error, then error code "3268 unhandled exception error"

I can reproduce it, Ironhyde ardboz in Deathpeak, approximately 22k,42k. not all of them have this bug, but the ones that do never respawn. just re-confirmed bug today
