Use the Icon of the Old One while standing on/close to the Monolith.

Once done, travel back to High Pass and speak with N'rarch Fleshreaper.

But nothing happens when i click the item on the Monolith.

Go to Monolith and
click the Icon of the Old One item in quest bag.
